Please Note:

This form must be submitted to Council a prior to the event occurring. 

Should this application be successful, the applicant is required to complete and submit an acquittal report no longer than 4 weeks following the event.  Acquittal forms can be found in SmartyGrants, under the 'My Submissions' tab.   

Failure to submit an acquittal form will result in applicants being ineligible to receive any further Performance Achievement Grants.  

Eligibility Criteria

In accordance with the application guidelines, you are required to verify the following:

The applicant is a resident of the Mackay Regional Council area. * Required

The applicant is of amateur status. * Required

The relevant governing body recognises the event I am apply for as . * Required
Response required.
Clear the selected value for The relevant governing body recognises the event I am apply for as .
The event is being held outside the Local Government boundaries of Mackay, Whitsunday and Isaac. * Required
